PA-115A Objection Form for Real Property Assessment  Please read the Property Assessment Appeal Guide above for instructions.  It is strongly encourage that you discuss your assessment with the assessor prior to filing an objection which begins the more formal appeal process.  
PA-813 Request for Waiver of Board of Review (BOR) Hearing  To request a waiver of a hearing in front of the Board of Review and proceed to circuit court.
PA-814 Request to Testify by Telephone or Submit a Sworn Written Statement at the Board of Review (BOR)  To be completed if you are unable to physically appear before the Board of Review
